Client Referral
If you need, or know someone who may need, our services please let us know. You can click here to download the referral form. The form can then be mailed to 285 South St., Suite J San Luis Obispo, CA 93401, faxed to ATTN: Senior Services Program Director (805) 547-7029, or e-mailed to goodneighbor@wilshirehcs.org. You can also call and make a referral over the phone at (805) 547-7025 ext. 26.
Once a referral is received it is reviewed by the Program Director and assigned to the appropriate Case Manager. The Case Manager then arranges an in-home assessment visit and meets one-on-one with the potential client. If the Case Manager determines that needs of the client would be appropriately served by our program then the client is enrolled.
Services can then be requested by calling the case manager with a specific need. For example, Cindy needs a ride to her dentist appointment next week so she calls her case manager with the date, time, location, and duration of her appointment. Then her case manager passes the request on to the Service Coordinator who in turn searches for an available volunteer. Once a volunteer states s/he is able to accommodate the need then Cindy will receive a call from the Service Coordinator.
Volunteers are given the client’s phone number so they can call and confirm on the day before the appointment.
If no volunteers are available then Cindy will receive a call from the Service Coordinator with information on other services in the area such as Ride-On.
